Free self management courses for patients with long term conditions

Self management uk is a charity offering self management support for people living with long-term health conditions.

They have a selection of free courses for patients with long term conditions such as diabetes, arthritis, chronic obstructive pulmonary disease (COPD) and heart disease.

The courses are for adults over 18 who feel they would benefit from learning self-management skills to support their care planning and future management of their condition. The courses are not suitable for patients with dementia, those who are house bound and patients whose mental health is not well controlled.

The courses run for six weeks and each session is 3 hours long including a refreshment break.

Locally this service has been commissioned by NELFT NHS Foundation Trust and is available to patients in the Basildon and Brentwood CCG locality. Patients can be referred by their GP/health professional and can also self-refer. See what patients say about self management uk's courses
